Filling Out the Generic Permanent Residence Application Form

This form has several areas you must fill out. Here is a quick tour through each set of items you must fill in:
  • Select the category of your application, as per the illustration below:

  • Select the language with which you are most comfortable for correspondence and interview:

  • Fill in your personal information as it appears on official documents. Remember, your given name is also known as your first name, and your family name is often called your surname:

  • Enter your marital status information. Note that the term common-law refers to a couple who has, for all intents and purposes, been living as spouses, but have not formalized their relationship:

  • State your knowledge of the official languages:


  • Enter your level of completion in formal education:


  • Describe your current occupation:


  • Enter your contact information:

  • Enter your official documentation (what you normally travel with) information:

On the second page of this form, you will find space for dependent information.

    Before question no. 1, just after the “language you prefer” section, there’s a box that needs to be filled in: “Visa Office requested for the process of your application”. What should be indicated there?

    In question no. 15, if the residential address is the same as the mailing address, should this be left blank?
